We Accept Most Major PPO Insurance Plans
NeuroHealth Arlington Heights accepts most major PPO insurance plans, including Blue Cross Blue Shield, Cigna, and UnitedHealthcare. We also accept certain Aetna plans for therapy services only and direct bill Medicare for eligible services.
Our team can help patients and families review benefit information before care begins. Direct billing is offered as a courtesy for eligible services and is subject to the terms of your individual insurance policy.
Note: NeuroHealth Arlington Heights does not accept Medicaid.
Coverage for neuropsychological and behavioral health services is determined by the insurance carrier and the specific insurance plan. Benefits may vary based on medical necessity criteria, referral or authorization requirements, deductibles, co-payments, co-insurance, network status, and service type. Also, insurance coverage for services is not a guarantee of claims processing or claims payment by the insurer for services rendered, and ultimately the patient and/or parent are responsible for payment on the patient’s account.
Conditions That We Evaluate and Treat
NeuroHealth provides neuropsychological evaluation and treatment for a variety of conditions including concussion, traumatic brain injury, other neurologic conditions, ADHD, learning-related concerns and learning disabilities, dyslexia, cognitive functioning, memory concerns, autism spectrum disorders, depression, anxiety, obsessive-compulsive concerns (OCD), mood and bipolar disorders, behavioral issues, and social concerns.
For older adults in Arlington Heights and surrounding communities, neuropsychological evaluation may support diagnostic clarification for conditions such as dementia and may help distinguish normal age-related changes from cognitive disorders that require further clinical attention. In addition, diagnostic services regarding fitness-to-drive, post-stroke functioning, and other behavioral and emotional changes that can occur with aging are provided.
Blue Cross Blue Shield Coverage for Neuropsychological Testing and Therapy Services
NeuroHealth Arlington Heights works with Blue Cross Blue Shield PPO plans for eligible neuropsychological testing and therapy services provided at the Arlington Heights practice.
When applicable, NeuroHealth Arlington Heights direct bills Blue Cross Blue Shield for services. Patient financial responsibility, including deductible, co-payment, co-insurance, authorization requirements, and any non-covered services, is determined by the terms of the individual plan.
UnitedHealthcare and Related Insurance Plans
NeuroHealth Arlington Heights accepts certain UnitedHealthcare commercial plans, United Behavioral Health plans, All Savers Insurance Company plans, and plans administered by United Medical Resources for eligible neuropsychological evaluations and therapy services.
Some of these plans may require pre-authorization or additional clinical documentation before testing or therapy services can begin. Health Savings Account or Flexible Spending Account funds may be applicable depending on the plan and the nature of the service.
Cigna Coverage for Neuropsychological Testing and Treatment
NeuroHealth Arlington Heights accepts certain Cigna plans for eligible neuropsychological testing and therapy services.
Cigna coverage varies by plan. Patients or parents should confirm whether pre-authorization is required, whether any limitations apply to covered sessions or testing services, and what out-of-pocket responsibility may apply under the plan.
Medicare Coverage for Neuropsychological Evaluations and Treatment
Medicare Part B may cover medically necessary outpatient neuropsychological evaluations or therapy services when applicable coverage criteria are met.
Medicare coverage may require a physician order, supporting documentation, and compliance with applicable medical necessity standards. NeuroHealth Arlington Heights has experience with Medicare billing and works directly with Medicare for claims submission.
Aetna Insurance Plans Accepted for Therapy Services Only
NeuroHealth Arlington Heights accepts certain Aetna plans for therapy services only at the Arlington Heights practice. Coverage for therapy depends on the individual plan and may be subject to medical necessity review, authorization requirements, deductible status, co-payment, co-insurance, and session limits.
Patients considering therapy through Aetna should review their benefits so they understand any plan requirements and expected financial responsibility. NeuroHealth Arlington Heights can assist with checking insurance benefits, but final coverage determinations are made by Aetna according to the terms of the policy.
Note: NeuroHealth Arlington Heights does not accept Aetna or Aetna Medicare for neuropsychological testing services.
Self-Pay Options and Additional Insurance Plans
Patients may choose to pay privately for neuropsychological services or therapy when insurance coverage is unavailable, limited, or not preferred. Self-pay arrangements may allow services to proceed without insurance authorization requirements; however, clinical recommendations and scheduling remain based on the nature of the requested service and the appropriateness of care.
NeuroHealth Arlington Heights accepts cash, checks, MasterCard, Visa, Discover, and American Express. Health Savings Account or Flexible Spending Account funds may be applicable for some services depending on plan rules and tax considerations. Patients should consult their plan administrator or tax advisor regarding the use of these funds.
Patients with insurance plans not listed above may contact NeuroHealth Arlington Heights at 847-558-7151 to discuss whether the practice can accept the plan. Availability of reimbursement depends on the insurance carrier and the patient’s plan terms.
Schedule Your Appointment
When patients or parents of children and adolescents schedule an initial appointment with NeuroHealth Arlington Heights, our administrative staff will review the insurance information that you provide to our office and assist with checking benefits for the accepted plans listed on this page. Please note that NeuroHealth checks benefits for patients as a courtesy and NeuroHealth cannot be responsible for inaccurate information that is provided by insurance companies to NeuroHealth regarding specific insurance plans. The insurance plan is a contract between the insurance carrier and the patient or parent and it is ultimately the responsibility of the patient or parent to know and understand their insurance benefits.
When contacting an insurance carrier directly, patients should have their insurance ID number and group number available. It may be helpful to ask specifically about coverage for neuropsychological testing or therapy, medical necessity requirements, pre-authorization, deductible status, co-payment or co-insurance amounts, and any limits on the number or type of covered services.
Although NeuroHealth Arlington Heights direct bills many major insurance plans and assists with checking benefits, insurance acceptance does not guarantee coverage, claims processing, or claims payment by the insurer for a specific service. While the patient’s individual policy determines covered services, exclusions, authorization requirements, and out-of-pocket responsibility, ultimately coverage and payment decisions are made by the insurance carrier.
NeuroHealth Arlington Heights has provided neuropsychological assessments, individual, family, and couples therapy, and educational advocacy services to patients and families in Arlington Heights and the greater Chicagoland area for more than 25 years.
